Coming to Valley Rehab for the first time?
Here's everything you need to know
to make your first visit pleasant.
WHAT TO EXPECT:
All new patients at Valley
Rehabilitation should expect to spend about an hour with the
therapist during their first appointment. During this time, the
therapist will obtain information from you by asking questions and
performing special evaluative tests. This information will be used
to build your customized treatment program, and at the end of your
treatment program the initial information will be used to determine
how much progress you have made.

information. To save time, please feel free to print these pages and
fill in as much information as possible before you come to your
first appointment. If you are unable to do this, please arrive at
our clinic approx 15 minutes before your scheduled time to complete
this paperwork. If you wish to read the unabridged version of Valley

WHAT TO BRING WITH YOU:

All health insurance
cards that you have
-
A list of all
prescription and non-prescription medications that you take

A physician’s
prescription if you have one. (If you would like to print our
prescription and take it to your physician, a link is provided
below.)
If you will be receiving
physical therapy for your leg or knee, it would be helpful to bring
shorts to your appointment. If you will be receiving physical
therapy for your arm or shoulder, it would be helpful to wear a
short sleeve or sleeveless shirt. Medical gowns are available if
necessary for treatment to the back or neck.
If your child has a walker, orthotics or braces, please bring them with
you to the evaluation. What to bring to later appointments will be
discussed during the evaluation.
